Initial Steps
• Scheduling Your Appointment: The first step is scheduling your appointment with our office. Once your appointment is scheduled, we will provide you with information about your upcoming visit and any paperwork that needs to be completed.
• New Patient Forms: Patients will be asked to complete their new patient forms electronically before their appointment. We encourage you to complete these forms in advance whenever possible so our team can review your information before you arrive. If you cannot complete your paperwork before your appointment, please arrive 15 minutes early to complete paperwork.
• Reminders: Our office will send out reminder texts and calls for your office visit.

Items to Bring
Patients need to bring:
• Photo ID
• Insurance cards
• Medication list

Pre-Visit Knowledge
• Parking: Parking is free, and each Koala location has a parking lot available for our patients.
• Arrival: We recommend arriving 15 minutes early so you have time to park, get settled, and check in for your appointment.
• Paperwork: If you have completed any forms or paperwork online before your appointment, please ensure they are submitted before you arrive. Completing your paperwork ahead of time makes the check-in process quick and easy and gives our team a chance to review your information before your visit.
• Overview: Your first visit is a time for us to get to know you, listen to your concerns, review your health history, and talk with you about your treatment options. Depending on the reason for your visit, we may need additional testing or records to help us better understand your needs.

First Impressions
• Check-In: When you arrive, you’ll be greeted by our friendly front office team, who will help you check in and ensure we have everything needed for your appointment.
• Provider Consultation: You’ll then meet with a member of our clinical team and your provider. We’ll take the time to listen to your concerns, review your history, discuss your symptoms, and answer any questions you may have.
• Treatment Planning: Before you leave, you’ll meet with your treatment planner to review your insurance benefits and go over your recommended treatment plan and next steps.

Our goal is to make you feel comfortable, informed, and confident in your care. We want you to feel comfortable asking questions and know exactly what to expect as you move forward with your treatment.
